I’m looking forward to working with you! Before your visit, please set up your account on the patient portal and complete your intake paperwork. The patient portal is secure and HIPAA compliant and will provide you access to your treatment plans, billing information, prescription information, appointment booking, communication with your provider, and more. Completing your paperwork before your visit gives me time to prepare in advance and maximizes your appointment time so that you are not completing paperwork during your visit.

If you are logging into the patient portal for the first time, click on the ‘New Sign Up’ button to set your initial password. A link will be sent to your email that will allow you to create a password and then sign on to the portal.
